We experienced a case of hypercalcitonemia probably caused by hyperplasia of C cells.<br> A 68-year-old wman was seen at the hospital because of a right thyroid tumor which was pointed out at a medical checkup. One tumor was palpated in the right upper lobe and another two tumors in the lower middle portion. Blood calcitonin level was as high as 485pg/ml. Gastrin loading test was conducted on her family members, but no abnormality was noted. Palpation and imagings indicated that the upper tumor was likely benign, and it was diagnosed as diffuse medullary carcinoama. The another two tumors were diagnosed as minimal papillary carcinoma because of calcification. Subtotal thyroidectomy and right-sided modified neck dissection were carried out. The excised upper tumor was a 2.1×1.5×1.2cm follicular adenoma and another two tumors with calcification were 0.7×0.6×0.5cm and 0.5×0.4×0.4cm papillary carcinoma. CEA and calcitonin were stained by enzyme antibody method. No staining was observed in the tumors, but there were calcitonin staining positive cells diffusedly or aggregately to form groups of 3 to 4 cells in the thyroid tissues surrounding the tumors. These observation indicated hyperplasia of C cells. Calcitonin level decreased to 17.3pg/ml on the 5th month after the operation.
