Metastatic thyroid tumors are rarely seen clinically, especially metastatic thyroid tumors from granular cell carcinoma of the kidney. This paper describes such a rare case of metastatic thyroid carcinoma from granular cell carcinoma of the kidney.<br> A 60-year-old man was seen at the hospital because of a swelling in the anterior neck and slight dyspnea which occurred abruptly one month before. There was a previous history of undergoing nephrectomy for a left renal cell carcinoma. This time fine needle aspiration cytology of the thyroid tumor suggested metastatic thyroid carcinoma. Total thyroidectomy and bilateral neck dissection were performed. Pathological findings revealed metastatic thyroid carcinoma from granular cell carcinoma of the kidney which was characterized by diffuse metastasis in the thyroid gland. The patient was followed on an ambulant basis while receiving additional chemotherapy.<br> The development of comprehensive medical care for cancers should increase the number of operable metastatic thyroid cancers. In such a background the importance of hearing the previous history and the value of needle aspiration cytology in making diagnosis should be emphasized.
