The treatment for pediatric inguinal hernia has been likely established, but the propriety of contralateral operation is still controversial.<br> From September 1974 to December 1993, a total of 2, 431 patients were operated on for inguinal hernia at the department. Of these patients, bilateral hernias appeared in 16.1% of males and in 28.3% of females. Contralateral operation was carried out in 8.5% of male and 19.9% of female patients with unilateral hernia who had silk sign in the contralateral site. Patent processus vaginalis of more than 1 cm in size was found in 78.2% of males and 94.5% in females. Fifty patients also had a contralateral lesion, in a frequency of 2.6%.<br> A variety of exploration methods for contralateral processus vaginalis have been attempted. Our surgical outcomes for contralateral lesions based on silk sign would have to present a basic diagnostic criterion.
