Abstract

We experienced a surgically treated patient with left adrenal metastasis one year and 9 months after an operation for a rectal cancer, with a favorable prognosis. The patient was a 60-year-old woman undergoing an abdominoperitoneal resection of the rectum for the rectal cancer. One year and two months after the operation, serum cartinoembryonic antigen (CEA) level gradually increased to 7.8ng/ml. Screening computed tomography revealed a heterogeneous mass in the left upper abdomen suggestive of adrenal metastasis. The tumor was successfully excised one year and nine months after the initial operation. The adrenal tumor was confirmed a metastasis from the rectal cancer by pathological examination. The CEA level decreased to normal range. The patient has remained well with no evidence of disease for nearly four years after the adrenectomy. The metastatic adrenal tumor following an operation for a rectal cancer should be positively excised, if the adrenal metastasis is dignosed as solitary and the primary lesion is well-controlled.
