A 58-year-old man under the treatment for chronic hepatitis on an ambulant basis was found to have a tumorous lesion in the anterior upper segment (S8) of the liver by abdominal ultrasonography. Dynamic CT and angiography revealed a hepatocellular carcinoma. Partial resection of the liver was performed. The resected specimen disclosed a small liver cancer 1.7cm in size.<br> One year and six months later, endoscopic examination revealed a small elevated lesion, in the upper intra-thoracic esophagus. Total resection of the thoracic esophagus was performed, the remnant stomach was conserved in terms of operative intervention, and the reconstruction was done by a right hemi-colon transplant through the antethoracic route. The histological diagnosis was poorly-differentiated squamous cell carcinoma, with cancer cells confined within submucosal layer of the esophagus. No lymph node metastasis or vascular invasion was noted. There have been no signs of recurrence.<br> The patient had an extremely rare double cancer. In the treatment of double cancer, an appropriate selection of surgical procedure which offers a cure as well as minimal surgical stress appears very important.
