Presacral mucus-secreting cysts in adults are rare. We experienced such a case in a 48-year-old woman. About 10 years before anorectal digital examination in the patient revealed a fingertip-sized and soft mass in the posterior rectal wall, but the mucosa surface of the rectum was normal. She had been followed up for 10 years. Recently she noticed an enlargement of the tumor. CT scan showed a wall marginated cystic, low-density mass in the presacral area. The tumor locating at about 3cm oral-side from the sphincter orni muscle was resected through a transsacral approach. The pathological diagnosis of the cyst was mucus-secreting presacral cyst. To our knowledge, this is the third report of an adult patient with mucus-secreting presarcral cyst in Japan.
