In our clinic, 382 cases of colorectal cancer have been surgically treated in a recent one decade (from Jan. 1983 to Dec. 1992). Tweleve (3.1%) of these cases were histologically confirmed mucinous carcinoma. The clinicopathological features of these mucinous carcinomas were compared with those of 357 differentiated adenocarcinomas.<br> The mean size of the mucinous carcinomas was larger than that of differentiated adenocarcinomas, and all mucinous carcinomas penetrated into the mucosal layer. The incidences of lymph node metastasis and peritoneal dissemination in the mucinous carcinomas were higher than those in differentiated adenocarcinomas, and there was no liver metastasis in the mucinous carcinomas. The cumulative five-year survival rate after curative resection of the mucinous carcinomas was 58.3%, versus 77.2% in differentiated adenocarcinomas, without significant difference. The mucinous carcinomas included metachronous multicentric carcinomas in a higher incidence compared with others (p<0.05).<br> The results indicate that mucinous carcinoma necessitates wide excision of surrounding tissues with extended lymph node dissection, and postoperatively, not only special attention to local recurrence, but also, surveilance for probable cancer in the residual large intestine should be required.
