Abstract

A 33-year-old woman was pointed out an upper abdominal mass on the 28th week of pregnancy. Giant hemangioma in the left hepatic lobe was most suspected by magnetic resonance imaging and ultrasonography. Cesarean section was performed on the 35th week of pregnancy. Hepatic hemangiosarcoma or malignant hemangiopericytoma was suspected by computed tomography and angiography which were undertaken 2 months later. Since intrahepatic metastases and bilateral lung metastases were detected, chemotherapy using cyclophosphamide, adriamycin, vincristine, 5-fluorouracil, cisplatin and interleukin2 was performed. But they were all ineffective, and 8 months after delivery, the patient died of brain metastases. Posthumous histopathological diagnosis was moderately differentiated hepatocellular carcinoma. Thus, we sometimes experience the difficulty in differentiation between hepatocellular carcinoma and vascular tumor such as hemangioma, hemangiosarcoma or hemangiopericytoma. Moreover, during pregnancy, limitation of invasive examination to the patients often prevent an accurate diagnosis and appropriate interventions.
