Abstract

A 50-year-old woman was referred to the hospital for unconsciousness. She was diagnosed as hypoglycemic syndrome with Whipple's triad. CT and angiography detected a large tumor (φ 3cm) in the body and multiple small tumors in the tails of the pancreas. Several examinations revealed that she had an insulinoma, parathyroid tumor, thyroid tumor and right adrenal gland tumor. She was diagnosed as MEA type I. Surgical treatment was performed on a two-step approach. At first, distal pancreatectomy, splenectomy and right adrenal gland tumor resection were performed. Secondary, subtotal parathyroidectomy and left hemithyroidectomy were added. A part of pancreas tumor was malignant insulinoma and thyroid tumor was papillary carcinoma and follicular carcinoma. Ten months later, lung cancer and breast cancer were detected. Right upper lobectomy and modified radical mastectomy were performed. We present the case of MEA type I combined with 4 cancers which is the first report in Japan.
