A 32-year-old woman was seen at another hospital because of an epigastralgia and was diagnosed as having a submucosal tumor of the stomach by endoscopy. The patient was admitted to the hospital for exploration and treatment. Endoscopic ultrasonography and abdominal computed tomography demonstrated that the lesion was a gastric cyst clearly defined from the pancreas. The cyst was punctured under the endoscopy and the collecting fluid was aspirated. Biochemical analysis of the fluid revealed following data; Amylase 37, 980IU/1, CA19-9 41, 392U/ml, and Elastase-1 120, 000ng/dl. Cytology of the fluid showed Class II indicating no malignancy. Finally we concluded that the cyst was a benign gastric cyst derived from an aberrant pancreas in the gastric wall. An endoscopic cystotomy was attempted, and epigastrlgia was erased. Four month after the cystotomy, the cyst started to grow again, then an endoscopic resection was carried out. There has been no recurrence of the symptoms nor any increase of remnant cyst, as of one year after the resection.<br> Gastric cysts appearing as gastric submucosal tumor are relatively rare. In this case reported here, endoscopic procedures were useful for getting a symptomatic remission.
