A 71-year-old female was admitted to the hospital because of abdominal distension. A mass about 20cm in diameter was palpated in the lower abdomen. Plain abdominal X-ray film showed a lowered permeability in the entire lower abdomen. An overain tumor was suspected on CT of the abdomen and the patient was operated on. Upon operation the ovary and uterus were normal and the mass was extraluminally growing submucosal tumor of the stomach, Partial gastrectomy was performed. The resected tumor was 5 kg in weight, and pathologic diangosis was leiomyosarcoma. The tumor grew extragastrically with few gastrointestinal symptoms and the patient was so fat which made her disregard abdominal distension as a meaningful sign. So the tumor was let alone to become giant, leading to delayed diagnosis. Moreover, extragastric growth and huge size of the tumor disturbed to identify the origin preoperatively.
