This paper describes a very rare case of basal cel adenocarcinoma of the parotid gland in an 82-year-old woman. The patient underwent an extirpation of a tumor of the left parotid gland in June 1985, which was diagnosed with basal cell adenoma. A swelling in the same region started to enlarge in October 1990. The patient was operated on under a suspecion of recurrence in October 1992, when intraoperative frozen section diagnosis revealed basal cell adenocarcinoma. Total excision of the left parotid gland was carried out. There has been no sign of recurrence, as of about 2 years after the operation. Rereview of the specimen at the initial operation also offered a diagnosis of basal cell adenocarcinoma. Basal cell adenocarcinoma which has been defined as a disease in the so-called 1990's WHO new classification is a rare entity and is defied as that of low grade malignancy. The disease, however, recurred in this patient. It is emphasized that minute pathological examination and appropriate selection of opertive procedure are needed for the disease.
