Therapeutic advantages of the thoracoscopic thoracic spranchnicectomy (TTS in short) for intractable pain relief in the chronic pancreatitis are reported.<br> During last two years, TTS procedure was performed on seven patients to denervate the pancreas of sensory afferents. Radiographic diagnosis of the pancreatic duct revealed a dilated type in two cases and nondilated type in five. The sympathetic chain of symptomatic side between Th 5-Th 9 was resected and electrocoagulated. If the pain relief after the first TTS was not sufficient, additional sympatectomy was performed on the other side of the patient.<br> In all patients, early postoperative course was almost uneventful and immediate pain relief was possible after TTS. Three patients, however, developed minor inter costal neuralgia and two patients experienced recurrence of the epigasric pain which was smaller in its intensity and extent compaired with preoperative pain. Six out of 7 patients successfully returned to their preoperative works or life style.These patients have been free from any symptom up to the present, at a maximum follow up of 23 months. None of these patients showed postoperative deterioration of the endocrine or exocrine function of the pancreas.<br> TTS had been proven to be a safe and reliable procedure for the pain relief of chronic pancreatitis and at the same time. TTS takes an advantage over other surgical procedures as a minimal invasive treatment.
