A 75-year-old woman was admitted to the hospital because of an epigastric pain. An upper gastro intestinal series showed a paraesophagial hiatal hernia. And a 6cm irregular large ulceration with round wall was seen in the hiatal hernia. CT scan showed the thickness of the gastric wall in the thoracic cavity. In the endoscopic examination, reflux esophagitis was not recognized. The patient was diagnosed as having a learge type 2 carcinoma on the greater curuvatur of the upper gastric body. A total gasrectomy with D2 lymph node dissection was performed. The tumor was 7cm in diameter and located most of the part of the herniated portion of the stomach. Microscopic examination of the tumor revealed poorly differentiated adenocarcinoma that infiltrated to the subserosal layer, without lymph node metastasis. Two years later the patient died of recurrence of the disease. Numerous reports can be seen in the Western literature on the assosiation of the diaphragmatic hiatal hernia and gastric cancer. However, almost all hernias were of sliding or short esophagus type and a gastric carcinoma within a paraesophageal hernia was rare. A total of four cases have been reported in the Japanese and foreign literature so far.
