Abstract

We experienced two cases of carcinoma of the small intestine where is an uncommon site for gastrointestinal carcinoma.<br> Case 1: A 47-year-old man was seen at the hospital because of an abdominal pain. He was diagnosed as intussusception by abdominal CT. Replacement by enema was tried, but it was impossible. The patient was operated on. Upon operation, we recognized that an epithelial tumor at about 130 cm from Bauhin's valve caused it. Partial excision of the small intestine with the mesenteric lymph nodes was performed. The pathological findings revealed that the resected tumor was well differentiated adenocarcinoma with the invasion of depth within subserosa, lymphatic vessel invasion and a regional lymph node metastasis.<br> Case 2: A 56-year-old man was seen at the hospital because of melena. A colonofiberscopy showed an edematous Bauhin's valve and a mass lesion at the end of the ileum. Angiography of the superior mesenteric artery visualized the tumor with hypervascularity at the end of the ileum. A malignant lymphoma was strongly suspected and a hemicolectomy was carried out. Pathologically it was advanced adenocarcinoma with poorly differentiation, depth invasion exposed to serosa, lymphatic or blood vessel invasion and lots of regional lymph node metastases.
