Abstract
Pyriform sinus fistula is relatively rare in neonates. Recently we experienced a neonate with pyrifrom sinus fistula presented with respiratory distress due to an enlarging tumor in the left region of the neck.<br> A 4-day-old girl was admitted to the hospital because of fever and a mass in the left region of the nect. On the fifth day she became respiratory distress, and was intubated. A plain radiography showed an air-containing mass with fluid level on the left side of the neck. Helical CT with 3-dimensional images revealed the distribution of a large cystic mass, and contrast study through the cyst showed a communication between the left pyriform sinus and the cyst. At the 20th day of age a complete excision of the cyst and fistula was performed. The postoperative course was uneventful.<br> In a review of the Japanse literature 13 babies with pyriform sinus fistula can be seen. Pyriform sinus fistula in neonates is charactarized by respiratory distress due to enlargement of the cyst and aircontaining cyst in a high frequency. Primary excision of the cyst and fistula is the treatment of choice and can be safely performed even in babies.
