Eighty-two resected cases of esophageal cancer without preoperative irradiation were subjected to a study. Except absolute non-curative resection (CO) cases and cases having other organ metastases (M1) intraoperatively. Three cases in which early recurrence was noted within about 3 months afte the operation were selected, and the severity of malignancy was investigated in terms of clinicopathological features as well as ploidy of nuclear DNA, amplification of hst-1 oncogene, and expression of p53 abnormal protein. All three patients had hematogenic recurrences in organs including the liver, lung, and bones within 4 months after the operation and died of cancer in one year. All patients were intraoperatively confirmed to have distant lymph node metastases in the neck and superior mediastinum, showing in the far advanced stage. Nuclear DNA ploidy was aneuploid in them, hst-1, oncogene amplification and p53 abnormal protein expression were confirmed in all patients. It is inferred that change in nuclear DNA ploidy and abnormality of the oncogene may reflect biological malignancy of esophageal cancers. A possibility that comprehensive evaluation of the malignancy as well as clinicopathological factors can a help to select therapeutic guideline appropriate for each case is suggested.
