Abstract

The paper describes an operated case of Crohn's disease with duodenal stenosis.<br> A 16-year-old male who was operated on for fistula ani 6 years before and diagnosed as ileocecal Crohn's disease 3 years before was operated on at the hospital after 10 months conservative therapy for duodenal stenosis.<br> Upper GI series showed a post-bulbar stenosis and thickened edematous mucosal folds. Gastroendoscopic examination demonstrated red nodural granular mucosa in the duodenum, but granuloma could not be detected in biopsied specimen.<br> Gastrojejunostomy for the duodenal obstruction and ileocecal resection for intestinal bleeding were performed. Resected specimen revealed longitudinal linear ulcers elsewhere in the ileum. Histopathologically any granuloma was not identified. But chronic inflammation, fibrosis, and giant cells, consistent with the diagnosis of Crohn's disease were seen.<br> Crohn's disease involving the duodenum is quite rare. Only 24 cases have been reported in Japan adding our case.
