Abstract

A 69-year-old woman was admitted to the hospital because of a lower abdominal pain. On admission, physical examination revealed a palpable mass in the right inguinal abdomen with rebound tenderness. Her body temperature was 38.4°C. Plain abdominal X-ray film revealed a tumor shadow with a ground glass appearance and some calcifications. Abdominal CT demonstrated a solid intrapelvic tumor of 12×8 cm in size. Laparotomy was performed with a clinical diagnosis of tumor of the appendix. A distended tumor of the appendix with a smooth surface was found. As no signs of malignancy, such as swelling of lymph nodes was see, ileocecal excision was carried out. The excised tumor was a simple mucocele with a thin wall, filled with a mucine-like substance of yellowish white color. The histopathological diagnosis was mucinous cystoadenoma of the appendix.<br> Preoperative diagnosis of appendical mucocele is very difficult. We have reviewed 64 cases (including the present case) of appendical mucocele reported in Japan within the past 10 years and discussed the different methods of diagnosis and treatment.
