So far it has been believed that the coverage with skin flap is needed for a defect of soft parts of the skin with exposed deep parts such as bone or tendon, because the graft survival is hardly attained. However, the patient has to make a great sacrifice for donating a skin flap from own body by leaving an agry scar. In this paper our experience with a case of intractable ulcer with exposed tendon which was successfully treated by using an artificial dermis is presented. together with some notes.<br> A 35-year-old man had an infection to a donated site after making a free dorsal frap of foot to form an intractabl ulcer with exposed tendon. For the ulcer a double-layered artificial dermis (Pelnac™) made by Gunze & Co. was grafted. Then, thick-split graft of 8/1000 inch from the thigh was done above dermis-like tissue construced 3 months after the grafting. A good graft survival was able to be attained. There has been no contracture, as of 6 months after the grafting. Further, this procedure permitted the least sucrifice for the donor site.
