Primary cancer of the small intestine is found in a relatively low incidence among gastrointestinal cancers. It is hardly detected in an early stage because of lacked specific clinical manifestations, for that, the prognosis is poor. A 54-year-old woman was admitted to the hospital because of nausea and abdominal discomfort. Serum CA19-9 level was high, 49.9U/ml. Upper gastrointestinal series revealed a stenosis due to a tumor in the jejunum about 50cm anal side from the Treitzs ligament. Selective SMA angiogram showed mild hypervascularity. Resection of the jejunum with lymph node dissection was performed under a diagnosis of jejunal cancer. Histologically, it was well differentiated adenocarcinoma with papillary growth, and cancer cells infiltrated into the subserosal layer. Serum CA19-9 level decreased after the operation, and immunoperoxidase stain using anti-CA19-9 was positive in the tumor cells. CA19-9 producting cancer of the small bowel is rare. Based on this experience, CA19-9 seems to be useful in detecting small bowel cancer and following up the clinical course.
