A rare case of Paget's disease of the breast 30 years after an augmentation mammaplasty is reported. A 54-year-old woman who had received an injection of silicone-fluid into the breast 30 years before was referred to the hospital because of eczematous lesion on the nipple of the right breast. No mass was found by palpation, mammography and ultrasonography. Scrape cytology of the nipple revealed Paget cells.<br> As she strongly hoped, a partial mastectomy including the eczematous lesion was performed. Histological examination revealed noninvasive ductal carcinima and siliconoma due to the augmentation mammoplasty, in addition to intraepidermal Paget cells. Although the definite guidline for the treatment of Paget's disease has not been established as yet, it is important for us to follow up the patient carefully from now on.
