Since 1993 when transanal endoscopic microsurgery (TEM), a new system for local resection of rectal tumor, developed in Germany by Buess and his coworkers was introduced, it has been increasingly used in Japan.<br> In 1994 from January to July, we treated four cases of rectal tumor with video-assisted TEM. There were three cases of early rectal carcinoma and one case of villous adenoma in three men and a women. The maximam diameter of each tumor ranged from 10mm to 25mm. The highest lesion resected was located 10cm from the anal verge. No large amount of bleeding was encountered intraoperatively. The operative time ranged from 35 minutes to 45 minutes (mean 42 minutes). Mean hospital stay was as short as about nine days on the average.<br> TEM is an alternative to a transsacral or transabdominal approach, with sufficient surgical margin and fewer complications. It is emerged as a minimally invasive and radical mean for local excision of rectal tumors.
