Abstract

Acquired fistulas between the esophagus and tracheobronchial tree are usually associated with malignant disease. We experienced a case of acquired esophagobronchial fistula probably caused by respiratory infection. A 60-year-old man was admitted to the hospital with a diagnosis of esophagobronchial fistula which was made at another hospital by upper gastrointestinal series. There was a previous history of repeated pulmonary infection which needed frequent hospitalizations for these 13 years. After admission an esophagoscopy revealed a fistula in the left wall of the intrathoracic middle portion of the esophagus. Bronchoscopy showed redness, swelling, and stenosed lumens in trees in the left lower lobe area of the lung. These findings and his episodes of pulmonary infection confirmed the diagnosis of esophagobronchial fistula and the patient was operated on. Upon operation, a fibrous fistulous tract with the left lower lobe of the lung was successfully removed despite the severe inflammatory adhesion between lung and pleura, and around the middle portion of the esophagus. The pathogenesis in the case and the treatment of acquired benign esophagobronchial fistula are discussed in this paper.
