Abstract

We experienced a patient with multiple ulcers in the terminal ileum, who was emergently operated on with a diagnosis of generalized peritonitis.<br> A 29-year-old man was admitted to the hospital because of high fever and severe abdominal pain on January 13, 1988. An abdominal X-ray film revealed a dilated small intestinal loop. Emergency operation was pereformed with a diagnosis of generalized peritonitis due to perforation of the small intestine. On laparotomy the entire small intestine was edematous and two perforated ulcers 5-8mm in diameter were found in the terminal ileum with the distance of 50cm and 80cm from the ileum end, respectively. The ileum with the length of approximately 150cm was resected. Resected fresh specimen involved six perforated ulcer lesions and multiple ulcers in the terminal ileum. The patient was diagnosed as having perforated ileal ulcers originating from Behçet disease. Postoperative course was uneventful. No recurrent sign has been detected for 8 years and 8 months after the operation.
