The giant cell carcinoma of the pancreas is very uncommon and has a poor prognosis. This paper describes a patient with giant cell carcinoma of the pancreas who has been doing well without any sign of recurrence for more than 5 years after operation.<br> A 55-year-old woman was seen at the hospital because of general fatigue. With various kinds of examinations, a huge tumor was confirmed on the tail of pancreas. The tumor was almost completely resected. Pathological diagnosis was made as giant cell carcinoma of the pancreas. After the operation chemotherapy comprising of 4 courses of epi-ADM and one-year administration of FT was conducted. No evidence of recurrence has been detected for 5 years and she is now living well.<br> In a review of the Japanese literature, we were able to see 33 cases of giant cell carcinoma of the pancreas so far. The disease was common in men and the tumors were predominantly located in the body and tail of the pancreas. Abnormal laboratory findings included increase in ESR, CRP, and CA19-9. Of 22 cases which were described about their lives and death, 17 patients died within one year after operation and five patients are alive. Although the prognosis of the diease is poor, all these alive patients had osteoclast type giant cell carcinoma (there were eight cases of this histological type out of 22 cases). So we can expect a good prognosis for patients with osteoclast type giant cell carcinoma, if a curative resection is possible.
