Abstract

Methicillin-resistant staphylococcus aureus (MRSA) is one of the serious complications occurring after gastrointestinal surgery. We experienced a case of postoperative enterocolitis due to MRSA. The patient was a 60-year-old man with rectal cancer. Severe diarrhea, high fever and massive bleeding developed on the 3rd day after a radical operation for rectal cancer and MRSA in the feces was confirmed by bacteriological examination. MRSA disappeared after the administration of Vancomycin and Imipenem/Cilastatin, but bloody stools due to enterocolitis developed. Finally, the patient died of multiple organ failure. A low echogenic lesion in the liver detected on a preopertive sonogram was confirmed as an MRSA abscess at autopsy, suggesting that this patient was an MRSA carrier. Hemorrhagic enterocolitis caused by MRSA is not common but its mortality rate is high. It is important therefore to make a full preoperative examination considering MRSA carriers and to select appropriate antibiotics for postoperative use.
