Abstract

We experienced a case of malignant ileal lymphoma causing intussusception in which the tumor was resected by laparoscopy assisted right hemicolectomy.<br> A 74-year-old woman was admitted to the hospital because of right lower abdominal pain and bloody stool lasting for one month. On admission, a fist-sized tumor was palpated in the right flank. Barium enema revealed a cupshaped defect in the right transverse colon. The intussusception was reduced by air inflation where the tumor shadow was shown in the cecum. Endoscopy revealed the elevated tumor with an uneven surface in the ileocecum region. She was diagnosed as having the intussusception due to the tumor in the ileocecum. To reduce surgical risk, a laparoscopy assisted surgery was applied. Laparoscopic findings showed an infant fist-sized tumor at the ileocecal region and enlargement of regional lymph nodes. Needle aspiration biopsy suggested a malignat lymphoma and laparoscopy assisted right hemicolectomy was carried out. A 7.0×5.5×1.5cm tumor was located in the ileum near the Bauhir valve in the resected specimen. Pathological diagnosis was malignant lymphoma, diffuse, large cell, B-cell type, se, and n1(+).
