Abstract

当科てんかん専門外来受診患者16例を対象に, フェニトインのMichaelis-Menten係数からみた薬物動態と薬物代謝酵素CYP2C19遺伝子多型の関係を検討した. その結果, (S)-メフェニトインのPoor Metabolizer (遺伝子型<I>m</I><SUB>1</SUB>/<I>m</I><SUB>1</SUB>, <I>m</I><SUB>2</SUB>/<I>m</I><SUB>2</SUB>) とExtensive Metabolizer (<I>m</I><SUB>1</SUB>/<I>wt</I>, <I>m</I><SUB>2</SUB>/<I>wt</I>, <I>wt</I>/<I>wt</I>) の両群間にKmおよびVmax値の有意差は認めなかったが, <I>m</I><SUB>2</SUB>遺伝子を持つ患者群 (<I>m</I><SUB>2</SUB>/<I>m</I><SUB>2</SUB>, <I>m</I><SUB>2</SUB>/<I>wt</I>) は, それを持たない患者群 (<I>m</I><SUB>1</SUB>/<I>m</I><SUB>1</SUB>, <I>m</I><SUB>1</SUB>/<I>wt</I>, <I>wt</I>/<I>wt</I>) と比較して, Km値, Vmax値が有意に低く, フェニトインの代謝異常に関与することが示唆された.この<I>m</I><SUB>2</SUB>, 遺伝子は日本人に特有の遺伝子変異とされることより, フェニトイン薬物動態の人種差について考察を加えた.

The relationship between the phenytoin pharmacokinetics by the Michaelis-Menten equation and CYP2C19 genotype were investigated in 16 Japanese epileptic patients.<BR>On comparison between the poor and extensive metabolizers of (S) -mephenytoin, there was no difference in the Michaelis-Menten parameters. But in the patients with the <I>m</I><SUB>2</SUB> allele of CYP2C19, the Km and Vmax value were significantly lower than those without the <I>m</I><SUB>2</SUB> allele.<BR>It is possible that phenytoin hypometabolism may be effected by the <I>m</I><SUB>2</SUB> allele of CYP2C19.
