Abstract

睡眠で自然に消失する非痙攣性てんかん発作重積状態 (NCSE) のみを示す2例を報告した。(1) NCSEのみを反復、(2) NCSEは睡眠で自然に消失、(3) 痙攣発作既往なし、(4) 非発作時脳波は前頭部優位の棘・多棘徐波が主、(5) 発作時脳波は全般性棘・多棘徐波結合の連続、(6) 脳画像は正常、であった。2例のNCSE時は軽度の意識減損と運動の拙劣さのみで、感情変化や自動症はみられなかった。一般的にはNCSEは救急治療の対象になり、予後が不良な例が多いが、睡眠で改善することや抗てんかん薬によく反応したことから、今回の2例の予後は良好である可能性が高いと思われた。

Two patients having recurrent episodes with nonconvulsive status epilepticus (NCSE) which ceased spontaneously by sleep were reported. Clinical characteristics were as follows;(1) no other seizure type than recurrent NCSEs which ceased spontaneously by sleep, (2) frontal region dominant spike-waves were observed in the interictal EEG, and ictal EEGs showed diffuse spike-waves, and (4) MRI was normal in each patient. Mild impairment of consciousness with ataxia was the only symptom during NCSE. Automatism was not observed during NCSE. NCSE is usually an emergent status and the prognosis is not so well. However, the prognoses of the present patients may be well since their episodes ceased spontaneously by sleep and responded well to anti-epileptic drugs.
