Metastatic tumors to the tonsil are rare. This report presents a case of a patient with a metastasis in the left palatine tonsil from hepatocellular carcinoma (HCC), which was the first manifestation of the disease.<BR>A 65-year-old male complained of a mass that existed in the lower portion of the left palatine tonsil with a thin stalk. The biopsy showed that the mass was a metastasis lesion of HCC. Clinical examinations revealed that a tumor of 15 cm in diameter with a necrotic change existed in the liver and another metastasis was found in the stomach. Symptomatic treatment was performed, but he died after 5 months later.<BR>The clinical aspects of metastatic tonsillar tumor and a possible mechanism of the metastatic processes are discussed
