Abstract

症例は58歳,男性.左血性乳頭分泌を主訴に, 2000年5月当院初診した.触診では腫瘤は触知せず,左乳頭より単口性血性分泌を認めた.マンモグラフィで乳頭直下に淡い腫瘤陰影を認めたが,超音波検査では異常を認めなかった.乳管造影にて,拡張乳管内の不整陰影欠損像を認めた.分泌細胞診ではclass III,乳頭分泌CEAは200ng/mlであった.確定診断のためmicrodochectomyを施行.肉眼的に乳管内に約6×6mmの乳頭状腫瘍を認めた.病理診断は,全体では非浸潤性乳管癌であるが被膜のごく一部に微小浸潤がみられ, noninvasive ductal carcinoma with focal microinvasionと結論した.標本上は完全切除であった.患者自身がこれ以上の治療を希望せず,術後1年6カ月の現在まで無再発生存中である.今回の症例は臨床的取り扱いとして非浸潤癌と同様であると考えられ,男性乳癌のなかでも貴重な症例であると思われた.

A rare case of noninvasive ductal carcinoma with focal microinvasion in the duct wall of the male breast with nipple discharge is reported. A 58-year-old man was seen at the hospital because of bloody nipple discharge in the left breast without palpable tumor. A mammography revealed a small pale tumor shadow, but there were no particular findings on ultrasonography. A secretion cytology examination revealed class III and CEA level of the discharge was below 200ng/ml. Ductgraphy demonstrated a dilation of the duct and intraductal filling defects. Microdochectomy was conducted to confirm the diagnosis. Macroscopically, a 6×6mm papillary tumor in the duct was seen. The pathological examination showed that noninvasive ductal carcinoma had a focal microinvasion in the only duct wall and the surgical margin was negative. No additional treatment was performed according to the patient's hope. He has been free from the disease, as of 18 months after the excision.
