Benign tumors may show sometimes a malignant change during a long-term follow up period and/or multiple surgical treatments for their recurrences. However, such cases of malignant changes from benign lipoma have been rarely reported.<br> We report here a case of benign lipoma which showed a malignant change to liposarcoma after totally six surgical extirpations for their recurrent tumors. An 80-year-old woman underwent the sixth total extirpation for a rapidly growing recurrent tumor on the thorax in the hospital in July 2000. It was well-differentiated liposarcoma. During the past 33 years starting in 1967, the patient had five recurrent benign lipomas and eventually the six recurrent tumor with a malignant change to liposarcoma. From a pathohistological standpoint, the first matured lipoma turned to a spindle cell lipoma which had a welldeveloped fibrous region with spindle cells but had neither lipoblasts nor mitotic cells. Next a few lipoblasts and atypical giant cells appeared in the tumor. And at last the tumor was identified clearly as liposarcoma.
