Twenty-one years have elapsed since Dr. Barry Marshall succeeded in culturing Helicobacter pylori for the first time in the world, and 20 years have elapsed since Dr. Hiroyuki Inoue, who was a post-graduated student in our department, cultured it for the first time in Japan. For this reason, 2001 was a special year for our H. pylori research. I was privileged with the honor of holding the 61st annual meeting of Japan Gastroenterological Endoscopy Society in Kobe, the first annual meeting in the 21st century, in this special year, and spoke of these 20 years of our H. pylori research in the president lecture. I am deeply grateful to Prof. Hirofumi Niwa, who is the president of Japan Gastroenterological Endoscopy Society and Prof. Shigeru Okuda, who is a man of experience in this field and who presided my lecture, for their kindness. Prof. Takashi Nakamura, an executive editor of this journal has recommended we write a review of our research. We will describe our work (and other results) for the 20 years, mainly focused on animal models and analysis of gene and LPS (Lipid A) in H. pylori. The contents of this paper are : 1. The history of our research 2. The bacterial characteristics of H. pylori 3. The infectious experiment using monkey 4. Bacterial factors including toxin and host factors including cytokines 5. The infectious route focused on childhood infection 6. The outcome (occurrence, relapse) of peptic ulcer associated with H. pylori 7. Mongolian gerbil model 8. Problems of eradication therapy. We do not mention gastric carcinogenesis and MALT lymphoma, because we do not have an established theory yet.
