Abstract

症例は58歳，男性．上腹部痛を主訴に当科外来を受診した,上部消化管内視鏡検査で胃癌と診断し，精査加療目的で入院した．第20病日に急激な呼吸困難，血圧低下で心タンポナーデで発症し，心嚢ドレナージ術を施行したが，第51病日に死亡した．病理解剖では，胃癌が横隔膜を介して心膜へ直接浸潤していた．胃癌による心臓転移はまれであるが，心タンポナーデの可能性も念頭におく必要があると考えられた．
