A 70-year-old woman was referred to the center for close exploration of an elevated lesion in the descending portion of duodenum, which was revealed by an upper gastrointestinal endoscopy at another hospital. Carcinoid tumor in the accessory papilla of duodenum was diagnosed by various examinations. No obvious metastasis was observed, however, images visualized that the tumor was larger than 20mm in maximal diameter which might possibly have micro metastasis. Therefore, surgical treatment was selected. Pathohistological findings demonstrated that the tumor was a carcinoid tumor with the maximum diameter of 15mm localized within submucosa. Besides, six out of fourteen lymph nodes harvested were positive. Including the present case, there have been seven cases of carcinoid tumor that developed in the accessory papilla of duodenum in the Japanese literature. Of these seven cases, regardless of diameter of the tumor, metastasis occurred in a high incidence. We think that the pancreatoduodenectomy with regional lymph node is adequate.
