- Long Term Survivors after Local Resection of Ampullary Carcinoma. Report of Two Cases.

Pancreaticoduodenectomy (PD) is generally the current standard operation for patients with ampullary carcinoma. However, this procedure is not always a good indication for the elderly patients or those with severe complications. We report two patients with ampullary carcinoma undergone a local resection and survive for three years without recurrence. Patient 1 was a 71-year-old woman who had undergone a subtotal esophagectomy with reconstruction by using a gastric tube for esophageal cancer three years earlier. Patient 2 was a 70-year-old woman who was under treatment with prednisolone for sarcoidosis and vasculitis syndrome. Both patients were thought not to be candidates for PD, and their ampullary carcinomas were localized and no metastasis including lymph nodes was found by preoperative abdominal CT scans. Lymph nodes are examined during operation and no metastasis was demonstrated. A small opening was made at the second portion of duodenum. Local resection of the ampulla including the tumor was made. The common bile duct and main pancreatic duct were separately anastomosed to the duodenum. Postoperative recovery was uneventful. Both patients are living for three years after the operation without any evidence of recurrence. Local resection of the ampulla is an excellent safe procedure from the standpoint of less surgical invasion and hence its indication must be considered according to each patient's condition in future.
