A 43-year-old woman with very short bowel syndrome was referred to the clinic because of cough. She had been cared with enteral nutrition (EN) and intermittent home parenteral nutrition (HPN). On admission, she was diagnosed as having pneumonia and received anti-biotics and intensive nutritional therapy. About 3 weeks after admission, X-ray showed cardiomegaly and an echocardiography revealed AR, MR and TR with vegetations. Multiple valves disease and cardiac cachexia due to IE caused by central venous catheter infection were diagnosed. She was treated with MVR, AVR and TAP and cared in conjunction with EN and PN. Diarrhea was successfully controlled by the high residual diet. She was discharged 4 months after surgery and has been well for 4 years after the operation. We suggest that appropriate therapy as well as perioperative intensive nutritional care is important for cardiac cachexia.
