Perforation of a duodenal diverticulum is very rare, with most cases reported requiring emergency surgery. A correct preoperative diagnosis is made in a few cases. A 59-year-old woman reported severe sudden pain in the paraumbilicus to the back, which was immediately relieved by analgesics, only to recur at midnight, so she admitted the next morning. Abdominal computed tomography (CT) showed pneumo-retroperitoneum and fluid collection surrounding the pancreatic head and right kidney. Under suspicion of duodenal perforation, we conducted fluoroscopy by gastrographine, finding a perforating diverticulum in the third duodenal part. Since her general condition was good we chose conservative therapy with a continuous suction by a nasal drainage tube indwelling in the second duodenal part, intravenous hyperalimentation, administration of antibiotic, H2-blocker, and nafamostat mesilate. She recovered gradually and was discharged 41 days after admission. This is, to our knowledge, the first case report of spontaneous perforation of the duodenal diverticulum successfully treated with conservative therapy under correct diagnosis in Japan.
