A case of inflammatory fibroid polyp (IFP) of the small intestine presented with positive fecal occult blood test is reported.<br> A 42-year-old man who was pointed out positive reaction to fecal occult test visited another hospital, and was found to have Yamada-IV type polyp about 2cm in size in the cecum by a colonofiber scopy (CF). It was Group 1 at a biopsy. CF at the hospital revealed that the polyp was present in the terminal ileum and endoscopic resection was difficult. Fluoroscopic study of the small intestine visualized a tumor about 2cm in size with a regular margin and a smooth surface, situating about 10cm to the Bauhin valve. Abdominal angiography revealed no abnormalities. A resection of the small intestine was performed. Histopathologically, an unknown foreign body was seen in the center of the tumor which was surrounded by necrotic tissues. Around the necrotic tissues, fibroblasts, giant cells, and epitheloid cells appeared which were further surrounded by infiltration of inflammatory cells mainly composed of eosinophiles. IFP of the small intestine was diagnosed.<br> This paper presents the case with bibliographical comments on the disease.
