We report a case of acute mesenteric artery embolism which was improved by second look procedure after an embolectomy that permitted us to preserve a small bowel segment measuring 60cm. A 69-year-old man was admitted to the hospital because of sudden onset of severe abdominal pain. A contrastenhanced CT scan depciting no opacification of the superior mesenteric artery (SMA) suggested occlusion of the artery. Aorta angiogram demonstrated embolus completely occluding the trunk of SMA. Nine and a half hours after the onset of this complaint an emergency laparotomy was performed. Large segment of the intestine and a part of the ascending colon revealed ischemic change. We performed an embolectomy of the SMA. About 24 hours after the embolectomy, second look procedure was performed, when a 60cm segment of the intestine was recoverd. Bowel resection of large segment and a partial colectomy were carried out. The postoperative course was uneventful, and the patient was discharged from the hospital 97 days after the surgery. Now, he can keep a good QOL with usual food under evacuation control. Second look procedure after an embolectomy appears useful for evaluation of the bowel viability. We conclude that the second look procedure could be recommended as one of the treatment options for embolus completely occluding the trunk of the SMA which is unresponsive to interventional thrombolysis treatment.
