Carcinoma in pleomorphic adenoma is a rare malignant salivary gland tumor, which comprises approximately 4.6 % of all malignant salivary gland tumors. The carcinomatous components in those tumors are mainly composed of undifferentiated carcinoma and adenocarcinoma, but it is reported that other carcinomas could occur in pleomorphic adenoma. In this report, an extremely rare case of carcinoma in pleomorphic adenoma that had a carcinomatous area composed of malignant myoepithelioma was presented.<BR>A 56-year-old female was visited our hospital with the chief complaint of a left soft palate swelling on May 27, 1994. The tumor had been previously excised at an other hospital, when the patient was 15 and 54 years old. The previous pathological diagnosis was pleomorphic adenoma at 54 years old. Based on the clinical diagnosis of salivary gland tumor, surgical operation was performed on June 14. The tumor microscopically consisted of two distinct components. One showed typical pleomorphic adenoma and the other revealed solid or cordlike proliferation of spindle-shaped or clear cells. These solid cells were immunohistochemically positive for S-100 and PK and had diastase-soluble PAS-positive granules in their cytoplasm, which indicated that the tumor was malignant myoepithelioma arising in pleomorphic adenoma. The patient had no local recurrence or distant metastasis for three years after surgical operation.
