We investigated 18 knees in 14 patients who had to have patellar replacements after total knee arthroplasty without patellar resurfacing, on the following points.
1) Does patello-femoral joint pain when standing up, walking, and going up or down stairs affect the ADL of patients?
2) The type of rheumatoid arthritis (mutilating type or non-mutilating type)
3) The form of the patella (Kuga classification)
4) Deviation and inclination degree of the patella
5) LT/LP (Insall-Salvati) ratio
All patients felt patellofemoral pain without patellar resurfacing. The result of classification of patellar form was that 10 were of the thick type (III b, IVb) (56%), 7 were lateral deviational type ( III a, IVa) (39%), and 1 was mixed type (5%) . Both types were patellar burgure. One had better resurface the patella when the patient suffers mutilating arthritis, or non-mutilating arthritis if patellar deviation and inclination are severe.
