Abstract
We report on the perioperative management of hepatectomy in a 64-year-old female with polycythemia vera (PV) . Although she has been treated for PV with venesection and hydroxyurea for two years, her hematocrit was still high. She was successfully treated with additional venesection and hydroxyurea for 40 days prior to the surgery, and with gabexate mesilate and hydroxyurea after the surgery. Twenty surgical cases associated with PV have been reported in Japan, in which hemorrhage and thrombosis were major causes of postoperative death. Patients with PV require perioperative management in consideration of complications such as DIC and thrombosis.
