A 48-year-old woman diagnosed by endoscopy with a submucosal tumor in the lower esophagus was found in a barium study to have a filling defect shadow in the lower esophagus 35mm long. Endoscopic ultrasonography and computed tomography showed a hypo-echoic, low-density mass spirally surrounding the lower esophagus, leading to a diagnosis of submucosal tumor of the lower esophagus. After laparotomy, the lower esophagus was dissected and pulled, and found to be spirally surrounded by a tumor whose margin was sharp. The tumor was enucleated. Histopathologically, the finding report was a benign leiomyoma. The patient recovered fully, and a post-operative barium study showed no stenosis, transformation or reflux of the esophagus.<br> This case of spirally developed sub-circumferential leiomyoma of the lower esophagus suggests that if a tumor of the lower esophagus is highly suspected to be benign, enucleation may be firstly recommended to preserve esophagogastric junction functions.
