A 42-year-old woman was referred to our hospital, complaining of abdominal pain at menses. She had been diagnosed as having bowel obstruction previously, but her symptom had resolved spontaneously when her menses ended.<BR>Oesophago-gastro-duodenoscopy and colonofiberscopy demonstrated no remarkable findings. Pelvic magnetic resonance imaging and barium follow-through of the small bowel confirmed cystic lesions of both ovaries, and stenosis of the distant ileum. From the onset and course, she was diagnosed as having ileal endometriosis.<BR>Laparoscopic surgery was performed. There was no adhesion, and a mass of the distant ileum like a torsion was observed. Oophorectomy of the right ovary and ileocecal resection was carried out. There was stenosis in the distant ileum, and histopathology confirmed the diagnosis of intestinal endometriosis of the ileum.
