We present a case of malignant melanoma in the anorectal area in a 65-year-old woman who was admitted after detection of a rectal tumor by colonoscopy. Abdominoperineal resection of the rectum was carried out under the diagnosis of anorectal malignant melanoma. Microscopic features were as follows : malignant melanoma, 30×17 mm, sm3, ow (-), aw (-), ew (-), ly1, v1, n1. Malignant melanoma cells had melanin granules and demonstrated so-called pagetoid spread in stratified squamous epithelium ; however, the tumor cells did not infiltrate the surgical margin. Immunohistochemistry was positive for HMB-45 and 5100. Malignant melanoma rarely infiltrates with the pattern of pagetoiJ spread. Thus, wide resection is encouraged to ensure an adequate surgical margin.
