We studied the qualitative changes of high density lipoprotein (HDL) subclasses in patients with type 2 diabetes mellitus using 8.5% polyacrylamide gel electrophoresis. The normal control subjects' HDL mainly consisted of HDL2a with large particle sizes (41.4 ± 0.8% Mean ± SEM) and of HDL3a with smaller parti-cle sizes (21.3 ± 0.5%). HDL2b, 3b and 3c were minor components in normal subjects. No signi-ficant differences were found between different genders or ages. In patients with type 2 di-abetes mellitus HDL2b and 2a (34.9 ± 1.2% p>0.001) decreased and HDL3a (24.2 ± 0.6% p>0.001) increased compared with those in nor-mal subjects. The HDL sizes of patients with type 2 diabetes mellitus were estimated as smal-ler than those of normal subjects. The amounts of smaller size HDL subclasses in serum were not correlated with diabetic markers, but rather with the polydisperse LDL, hypo-HDL cholesterolemia, hypertriglyceridemia and arteriosclerosis index. Therefore, it was considered that the measurement of HDL subclasses in patients with type 2 diabetes mellitus was useful in the prevention and management of arteriosclerosis.
