We report two operated cases of abdominal tuberculosis presented with acute abdomen in young men. Case 1: A 29-year-old man was admitted to the hospital because of right lower abdominal pain and fever. An emergency laparotomy was performed on suspicion of acute appendicitis and ileocecal abscess. A 5-cm, enlarged lymph node found in the ileocecal mesenterium was resected. Pathological examinations revealed epithelioid granulomas with caseation necrosis and the presence of acid-fast bacilli. Tuberculous lymphadenitis was diagnosed. Case 2: A 28-year-old man was admitted to the hospital because of severe abdominal pain and vomiting. An emergency laparotomy was performed on suspicion of strangulation of the intestine. A circular ulcer, resulting in severe stenosis, was found in the ileum 2m distal from the Treitz's ligament and a partial resection of the ileum was done. Pathological examinations revealed epithelioid granulomas without caseation necrosis and the presence of acid-fast bacilli. The diagnosis of intestinal tuberculosis was confirmed. Both patients were further treated with antituberculous drugs. The incidence of tuberculosis is still high in this country. It is important that physicians have awareness of tuberculosis as a potential source of acute abdomen in young people.
