This paper deals with a case of Crohn's disease in an elderly presented with intestinal obstruction.<br>A 74-year-old woman seen at the hospital because of abdominal pain and diarrhea was admitted with a diagnosis of intestinal obstruction. After admission decompression therapy by inserting a tube was started, but no remission was attained. Colonofiberscopy revealed extensive narrowing over the entire circumference of the transverse colon, and the patient was operated on. Upon laparotomy, there existed a narrowing from the hepatic flexure of the transverse colon to splenic flexure of the descending colon. No abnormalities were seen in the small intestine. To resect the narrowing portion of the colon, an extended right hemicolectomy was performed. Macroscopically the serosa became thickened and longitudinal ulcer was identified on the mucosa. Histopathologically, there were ulcer reaching to the submucosal layer and non-caseous epithelioid granuloma cells partially. Crohn's disease was definitely diagnosed.<br>Since Crohn's disease is a relatively rare entity in elderly people, this case is presented here, with some bibliographical comments.
